It is a series of medical examinations and tests that are performed to find the answer to the question of why a couple is unable to conceive even after 12 months of unprotected intercourse. These tests are recommended for both partners to know the underlying health issues affecting fertility.

Female Fertility Evaluation

The assessment is based on your age, present symptoms, medical history, cycle pattern, and previous reports.

  • Detailed History: Your age, duration of trying, menstrual pattern, prior pregnancies or miscarriages, past infections and surgeries (if any), and lifestyle choices are taken into consideration.
  • Physical and Pelvic Examination: Your weight, BMI, signs of hormonal imbalance (for example, excess hair growth or acne), or thyroid enlargement are evaluated. The uterus, cervix, and ovaries are checked for any issues.
  • Ovarian Reserve Tests: These are primary tests that include the Anti-Müllerian Hormone (AMH) (blood test), Antral Follicle Count (AFC) (ultrasound), and Follicle-Stimulating Hormone (FSH) (day 3 blood test). These are recommended for women over 35 or those facing infertility. 

       High AMH/High AFC indicate good ovarian reserve and vice versa. High FSH indicates poor ovarian reserve.

  • Transvaginal Ultrasound: Helps examine a woman's pelvic organs, including the uterus, ovaries, cervix, and dilated fallopian tubes. Check the presence of fibroids, polyps, congenital anomalies, cysts, and PCOS.
  • Hysterosalpingography (HSG): Helps analyse the shape of the uterus and the patency (openness) of the fallopian tube.
  • Sonohysterography: It is also known as saline infusion sonography. Sonohysterography is a procedure to look at the inside of the womb.
  • Hystero Contrast Sonography (HyCoSy): It is a transvaginal ultrasound used to evaluate fallopian tube patency (blockage) and the uterine cavity.
  • Hormonal Blood Tests: Measures hormonal balance such as FSH, LH, oestradiol, TSH, prolactin and progesterone.
  • Laparoscopy (if required): It is a minimally invasive procedure used to diagnose and treat endometriosis, adhesions, fibroids, or tubal problems.

Male Fertility Evaluation

A complete fertility evaluation always includes the male partner. Here are the tests that can be recommended:

  • Detailed Review of Medical History: Past lifestyle habits, surgeries or injuries, medications and medical conditions are taken into consideration.
  • Semen Analysis: It’s one of the first steps in detecting fertility issues. The semen sample is evaluated under a microscope to know the sperm count, sperm activity and shape.
  • Hormonal Tests: Male hormonal profiles are evaluated, which include FSH, LH, testosterone, and prolactin.
  • Additional Tests: In some cases, male partners can be recommended genetic screening and scrotal ultrasound.

At what age is peak Fertility?

Female fertility is best in the late 20s and remains relatively good up to the early 30s. There is no cutoff age for men; however, sperm quality generally can start to reduce around age 40 to 45 years.

At what age are 90% of a woman's eggs gone?

While it is unclear and may vary person to person, studies suggest that a woman's lifetime egg supply can be depleted faster after the mid-30s. But that does not mean that they cannot conceive; it just means the pools of eggs have shrunk.

Is 49% Sperm motility good?

Sperm motility of more than 40% is considered a good and normal result. The higher the percentage, the more sperm there are that can swim to meet the egg for fertilization.

Can I get Pregnant with 40% Sperm motility ?

Yes, 40% sperm motility means that 40% of sperm can efficiently swim and navigate the female reproductive tract to fertilize an egg. But chances of conception depend on the complete semen analysis (count, progressive motility, morphology).

Is a Female Fertility test painful?

No, but some blood tests and procedures can cause brief discomfort. HSG or HyCoSy can cause period‑like cramps for a few minutes; the pain is of short duration and manageable. A smaller number have severe pain, especially in the case of blocked fallopian tubes.
